HiTalent Recruitment is seeking a Specialist Case Manager to join a well established Not for Profit organisation. In this role, you will be responsible for overseeing a designated caseload, guiding clients from the initial referral and intake through to exit. You will work hands-on with clients, providing both structured case management and immediate support ensuring positive outcomes for clients at risk of homelessness

You will be responsible for:

• Conducting comprehensive intake assessment for each referral, thoroughly evaluating psychosocial, housing, health, and complex support needs

• Ensure that risk factors and support requirements are balanced to maintain program continuity and a stable environment for clients.

• Facilitate the development of adaptive and sustainable strategies, empowering clients to achieve clearly defined short-term and long-term goals.

• Monitor and regularly evaluate clients progress through regular reviews, ensuring goals remain achievable, sustainable, and aligned with their evolving needs

• Build and nurture relationships with community organisations, external support agencies, and internal housing services.

To be successful in this role, you will require: 

• Relevant tertiary qualification in Social Work, Community Services, or related mental health discipline.

• Minimum of 2 years demonstrated experience providing specialist and complex case management, ideally within mental health, homelessness, or community services sectors.

• Comprehensive understanding and substantial experience within the homelessness and housing support sector in NSW

• Proven ability to work under pressure, handle difficult situations, and manage a diverse caseload effectively.

• Experience in a not-for-profit organisation is desirable
