We are currently seeking a compassionate and capable Case Worker to join the Social Mission team. This is a meaningful opportunity to work within a client-led, recovery-oriented framework, supporting individuals impacted by mental health challenges, AOD and complex life circumstances.
You will deliver high-quality, person-centred case management services that empower clients to achieve their goals and support long-term recovery.
You will play a key role in assessment, care planning, referrals, education and group work, while working collaboratively with internal teams and external services.
Service Delivery & Practice
Deliver day-to-day case work, assessments, interventions, education and group programs within a recovery-oriented, client led framework
Develop, implement and review individual treatment and case plans
Maintain accurate case notes, reports and data in line with guidelines
Provide brief interventions and harm minimisation support
Contribute to continuous improvement and innovative service delivery
Collaboration & Networking
Build and maintain strong knowledge of referral pathways and community supports
Work collaboratively with internal teams and external stakeholders
Foster professional, respectful interagency relationships
You are a resilient and reflective practitioner who thrives in complex environments and is committed to holistic, person-centred care.
Qualifications & Experience
Minimum Diploma qualification in Community Services, Case Management or similar
Minimum 2 years’ experience working in recovery-focused or related settings
Demonstrated experience supporting individuals impacted by mental health challenges, substance use and other challenges
Strong understanding of trauma-informed and strengths-based practice
Sound computer literacy and strong administrative skills
Ability to manage unpredictable situations using de-escalation techniques
Current Driver’s Licence
Strong communication skills with the ability to engage diverse individuals and groups
A collaborative approach and ability to build trusted relationships
Cultural competence and commitment to inclusive, respectful practice
Ability to critically reflect on practice and continuously improve
National Police Check
