Mental Health Care Coordinator – Emerald

HiTalent Recruitment is seeking a committed and community-focused Mental Health Care Coordinator to join a well-respected Not-for-Profit organisation in Emerald. This service forms part of a large, collaborative program delivering accessible, mental health support across the region.

Are you a dedicated mental health professional who thrives in coordination, engagement, and collaborative care?

 In this position, you will play a pivotal role in supporting clients as they enter the service, providing brief interventions, coordinating care pathways, and ensuring seamless, person-centred support.

Responsibilites

  • Be the first point of contact for clients referred via the Head to Health Phone Line and other pathways.

  • Lead and coordinate each client’s care journey, ensuring smooth transitions and high-quality outcomes.

  • Provide early engagement, brief interventions, and aftercare support.

  • Work collaboratively within a multi-agency model, building strong relationships with internal teams and external partners.

  • Oversee care planning, service navigation, and transitions between service streams.

  • Engage with the lived-experience workforce and local hub partners to strengthen community support networks.

Qualifications & Registration

  • Tertiary qualifications in Psychology, Social Work, Mental Health Nursing, Counselling, or a related discipline.

  • Professional registration or membership (AHPRA, AASW, PACFA) – highly regarded.

Experience & Capabilities

  • Demonstrated experience in mental health and/or AOD settings.

  • Strong understanding of mental health systems, stepped-care approaches, and brief-intervention models.

  • Ability to work both independently and within a multidisciplinary team.

  • Proven collaboration skills with diverse services and professionals.

  • Experience supporting clients in community-based or recovery-oriented frameworks.
