HiTalent Recruitment is seeking a Specialist Case Manager to join a well established Not for Profit organisation, that works to enrich the lives of veterans and seniors. This role will be part of the Veteran Services program, and will support clients who are homeless or at risk of homelessness.

You will be responsible for:

• Conducting comprehensive intake assessment for each referral, thoroughly evaluating psychosocial, housing, health, and complex support needs

• Ensure that risk factors and support requirements are balanced to maintain program continuity and a stable environment for all participants.

• Develop individualised transitional support plans for veterans, employing a veteran centric, trauma-informed, strengths based, and recovery- oriented approach.

• Facilitate the development of adaptive and sustainable strategies, empowering veterans to achieve clearly defined short-term and long-term goals.

• Monitor and regularly evaluate veteran progress through regular reviews, ensuring goals remain achievable, sustainable, and aligned with their evolving needs

• Proactively support veterans' successful transition back into the community, maintaining continuity of care through outreach and follow-up case management

• Take reasonable steps to support safe and stable accommodation for veterans within the program.

To be successful in this role, you will require: 

• Relevant tertiary qualification (minimum bachelor’s degree) in Social Work, Community Services, or related mental health discipline.

• Minimum of 5 years demonstrated experience providing specialist and complex case management, ideally within mental health, veteran support, homelessness, or community services sectors.

• Comprehensive understanding and substantial experience within the homelessness and housing support sector in NSW

• Proven ability to work under pressure, handle difficult situations, and manage a diverse caseload effectively.

• Experience in a not-for-profit organisation is desirable

This is a full time, permanent opportunity offering $110,000 + super + salary packaging
