KBHAC is a survivor-led Aboriginal Community Controlled Organisation dedicated to supporting Stolen Generations Survivors, their families and Aboriginal communities through healing, truth-telling and culturally safe services.

About the Role
Reporting to the Program Manager, the Elder Care Connect Worker provides culturally safe, strengths-based support to Aboriginal older people, their families and carers.
You will assist clients to access aged care, health and community services, provide advocacy and case management, coordinate referrals and support clients to navigate My Aged Care and other relevant services.
The role focuses on promoting independence, improving wellbeing and strengthening community connections while working collaboratively with Aboriginal organisations, health providers and government agencies.
Regular community outreach across Sydney forms an important part of this position.
Key Responsibilities
Provide case management and practical support to Aboriginal Elders and their families.
Assist clients to access My Aged Care and other community services.
Develop person-centred support plans that promote independence and wellbeing.
Advocate on behalf of clients to improve access to services.
Coordinate referrals and liaise with health, aged care and community service providers.
Build strong relationships with Aboriginal Community Controlled Organisations and key stakeholders.
Deliver community outreach and promote available support services.
Maintain accurate case notes, documentation and client records.
Monitor client outcomes and review support plans as required.
Participate in team meetings, supervision and ongoing professional development.
What We're Looking For
Essential
Aboriginal and/or Torres Strait Islander person (Identified Position).
Relevant qualification in Community Services, Social Work, Welfare, Aged Care or another Human Services discipline, or equivalent experience.
Demonstrated experience working with Aboriginal communities.
Experience providing case management or care coordination.
Strong understanding of culturally safe and strengths-based practice.
Excellent communication, advocacy and stakeholder engagement skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Current NSW Driver's Licence.
Working With Children Check.
National Police Check.
Willingness to undertake community outreach across Sydney.
Highly Regarded
Experience supporting older people or working within the aged care sector.
Knowledge of My Aged Care and aged care services.
Experience working within an Aboriginal Community Controlled Organisation.
Understanding of trauma-informed practice.
Experience using client management systems.
Roster & Working Hours
This is a full-time permanent position based in Waterloo, Sydney NSW.
Working hours are Monday to Friday, 9:00 am – 5:00 pm (38 hours per week).
The role includes regular community outreach across Sydney to support Aboriginal Elders and strengthen community connections.
